Unison community support providers coordinate care for adults who are coping with a severe persistent mental illness. The CSP and consumer work together to pick and choose the Unison services that the consumer requires to work toward recovery. They continually evaluate the plan to ensure that it meets the consumer's changing needs and helps the consumer attain personal goals. The CSP also offers encouragement through one-on-one support, advocates for consumer rights, teaches skills for successful community living, coordinates care with family members and other service providers, and provides support during crisis.
